Title: Arresting Ebola Spread: Ring Vaccination Proves Effective in DRC
In the ongoing battle against deadly epidemics, public health responses must be swift and targeted. A recent study in the New England Journal of Medicine demonstrates the effectiveness of an innovative strategy to combat Ebola: ring vaccination. This approach, proven successful in Guinea, involves vaccinating not only those in direct contact with infected individuals, but also their contacts’ contacts.
Researchers in the Democratic Republic of Congo (DRC) evaluated the impact of this methodology during the 2018-2020 Ebola epidemic. They formed ‘rings’ around confirmed cases, offering the replicating rVSV-ZEBOV-GP vaccine to contacts and their networks within 21 days of symptom onset. By doing so, they aimed to mitigate the disease’s spread from its epicenter.
What did they find?
Between August 2018 and January 2020, 265,183 individuals were vaccinated. Among these, 102,515 were closely monitored for safety. The study revealed that 434 Ebola cases emerged, almost all within the first 9 days or between days 10 and 29 post-vaccination. Notably, only 22 additional cases occurred after day 29, during an average of 170 days of follow-up.
The key takeaway? The earlier control measures—including ring vaccination—were implemented after the Ebola case index’s onset, the more effectively the disease’s spread among contacts was curbed. By day 10, Ebola rates Plunged, with those still symptom-free exhibiting a 0.16% Ebola rate between days 10 and 29.
Comparisons with Guinea’s 2015 ring vaccination trial, where standard control measures were swiftly initiated but vaccinationwas delayed, underscored the strategy’s success. The DRC’s Ebola rate (0.16%) was eighth of that seen in Guinea (4.64%), validating the expedited vaccination approach’s benefit.
No significant safety concerns were raised regarding the vaccine.
Expanding Reach, Saving Lives
As the epidemic evolved, the study expanded its vaccination net to include children, women in late-stage pregnancy, and lactating mothers. Despite meticulous tracking of nearly 200,000 contacts or contact contacts, few additional Ebola cases were identified among the vaccinated post-intervention.
The single-dose rVSV-ZEBOV-GP vaccine, pre-qualified by the World Health Organization (WHO) and approved in various regions, including the EU and the US, proved both safe and effective. Moreover, it might have reduced mortality rates among those vaccinated, as evidenced by lower fatality rates in those diagnosed 10+ days post-vaccination.
