Avoiding high blood pressure, type 2 diabetes, and smoking during middle age can extend dementia-free survival by nearly 13 years, according to a medical study published in Neurology. Researchers analyzing decades of health data from NYU Langone Health found that maintaining these three vascular health factors significantly preserves long-term brain health.
Middle-Age Health Factors Linked to 13 Extra Years Free of Dementia
Maintaining specific cardiovascular health measures between the ages of 45 and 65 can ward off cognitive decline for well over a decade according to research published in the medical journal Neurology. Investigators at NYU Langone Health examined data from the Atherosclerosis Risk in Communities study, which began in 1986 and tracked participants across decades to evaluate midlife vascular health and subsequent dementia outcomes.

The analysis focused on 12,409 participants with an average age of 56 who were free of dementia at the outset. Researchers assessed subjects for three primary risk factors: high blood pressure, type 2 diabetes, and smoking habits. Following the cohort for an average of 26 years, the team recorded 3,008 cases of dementia and 5,238 deaths resulting from other causes before the onset of cognitive decline.
Participants who entered middle age free of all three conditions enjoyed a dramatic extension in cognitive health. Individuals with no midlife vascular risk factors lived an average of 13 more dementia-free survival years compared to those burdened by all three risk factors.
Breakdown of Dementia-Free Years by Risk Factor Count
The duration of cognitive health scaled directly with the number of managed risk factors. Subjects presenting with zero risk factors at the start of the study lived an average of 30.1 years free of dementia. Those managing a single risk factor retained an extra 26.3 dementia-free years, while individuals dealing with two risk factors averaged 21 additional years. Participants exhibiting all three risk factors at baseline averaged just over 17 years without dementia.

The study also highlighted demographic variations in dementia-free survival. Regardless of how many risk factors were present, women lived longer without dementia than men, and white participants recorded more dementia-free years than Black participants.

Dr Jacqui Hanley, head of research funding at Alzheimer’s Research UK, noted that framing the data around dementia-free years provides a practical perspective for patients evaluating long-term health choices.
