Moderna and Merck Melanoma Vaccine Succeeds in Phase 3 Trial

The late-stage study enrolled 1,137 patients with completely resected stage IIB-IV cutaneous melanoma. Earlier mid-stage five-year data presented in June showed that adding the vaccine to Keytruda cut the risk of cancer returning or death by 49%, and reduced the risk of distant metastasis or death by 59%, compared with Keytruda alone.

How the Personalized Vaccine Works

Unlike preventive vaccines designed to ward off infections, intismeran autogene is a therapeutic treatment administered to patients who already have cancer. The therapy is tailored specifically to each individual. After surgical removal of the melanoma, a sample of the patient’s tumor is analyzed to identify mutations unique to those cancer cells. Messenger RNA technology is then used to provide genetic instructions corresponding to selected tumor-specific neoantigens, which act as molecular flags distinguishing cancer cells from healthy tissue.

This customized jab trains the patient’s immune system—particularly T cells—to recognize and attack those specific targets. The approach combines two complementary strategies: the vaccine teaches the immune system what targets to look for, while pembrolizumab removes natural brakes that can prevent immune cells from attacking cancer.
