Researchers in Denmark analyzed PubMed, Embase, and Web of Science databases for studies published up to September 30, 2025. According to the meta-analysis led by Karimian and colleagues, the pooled prevalence of formaldehyde contact allergy across 1,347,638 dermatitis patients landed at 2.88%. Of those positive reactions, 41.57% were deemed clinically relevant to the patient’s skin condition.
The study revealed striking geographical disparities. North America recorded the highest prevalence of formaldehyde contact allergy at 6.8%. Europe followed at 2.3%, while Southeast Asia registered the lowest rate at 1.8%. Other regions also showed elevated numbers, including South America at 5.4%, Africa at 5.3%, and East Asia at 4.2%, though some regional estimates relied on fewer studies.

Quaternium-15 triggered the greatest clinical relevance among these preservatives. According to the findings, 55.67% of positive reactions to quaternium-15 were directly relevant to the patients’ dermatitis.
How Regulatory Policies Impact Contact Dermatitis Rates
Regulatory differences between countries offer a primary explanation for the stark geographical gaps in allergy rates. In the European Union, formaldehyde and quaternium-15 are prohibited in cosmetic products, alongside strict rules limiting other formaldehyde-releasing preservatives. The study authors noted that these interventions likely reduce population exposure and subsequent sensitisation, pointing to regulation as a key preventative strategy.
