Thailand is facing a critical liver disease crisis. Over 20 million Thais – nearly one-third of the population – are grappling with fatty liver disease. The northeast region is particularly affected by liver fluke, with infection rates reaching one in three residents. Left unchecked, these conditions can lead to cirrhosis, liver cancer, and life-threatening complications.
The Cost Barrier to Early Detection
A significant obstacle to effective treatment is the high cost of diagnostic tools. FibroScan, a non-invasive liver health scanner, costs between 2.5 to 7 million baht per unit. Only 17 public hospitals nationwide have access to this technology. Even when available, a single scan can cost 3,000-5,000 baht, placing it beyond the reach of many low-income patients. This lack of accessibility creates a critical gap in healthcare provision.
SmartLiva: AI-Powered Ultrasound Analysis for Affordable Liver Screening
Recognizing this challenge, two students from the Smart Materials Laboratory (SMART LAB) at King Mongkut’s University of Technology Thonburi (KMUTT) – Purinat Polasa (Nano), a third-year computer engineering student, and Chonlaya Kreu-wutikul (Wawa), a first-year student at the Field Robotics Institute (FIBO) – developed SmartLiva. This innovative system uses artificial intelligence to analyze ultrasound images, identifying liver pathologies, assessing fibrosis, and detecting fat accumulation.
How SmartLiva Works: Upgrading Traditional Ultrasound
SmartLiva functions as an “upgrade” to conventional black-and-white ultrasound imaging. Physicians upload images via a web interface, accessible on computers or tablets. The AI then enhances the image clarity, isolates the liver area for analysis, and highlights abnormalities using color-coding (e.g., red for the liver, yellow for fat). This visual aid makes it easier to identify issues quickly and accurately.
The AI simultaneously assesses three key factors: the stage of fibrosis (F0-F4), the presence of abnormalities like cysts or liver cancer, and the detection of liver flukes. The system processes the data and generates a report with accompanying images and explanations understandable to both medical professionals and patients. The AI was trained using a database of over 50,000 ultrasound images.
Accuracy and Speed: A Game Changer for Liver Diagnosis
“The results from SmartLiva are very promising. The system can analyze results with considerable accuracy, correctly assessing the stage of liver fibrosis in 92% of cases, identifying tissue abnormalities with 87% accuracy, and detecting liver flukes with 84% accuracy,” explains Wawa. Crucially, the entire process takes just 7 seconds, a significant reduction from the potentially lengthy wait times associated with traditional analysis. This speed and accuracy can improve early diagnosis and treatment outcomes.
Reducing Healthcare Costs and Improving Access
SmartLiva dramatically reduces the cost of liver disease screening, lowering it by 77% – from thousands of baht to just 200-300 baht. This price point makes the test more accessible to those covered by social security or general health insurance. The system’s web-based application is also user-friendly and compatible with computers, tablets, and mobile phones, enabling smaller hospitals without FibroScan machines to screen patients for early-stage liver disease.
Future Developments: Explainable AI and Portable Diagnostics
The developers are planning to enhance SmartLiva with Explainable AI (XAI), providing detailed explanations of each diagnostic step. They are also developing “HepaSage,” an intelligent assistant that summarizes findings into easily understandable reports for both patients and healthcare providers. Future plans include creating a portable version of the system for use in clinics and remote areas, and expanding the AI’s capabilities to other time-sensitive conditions, such as detecting blood clots in the brains of stroke patients.
