At the highest dose, average weight loss reached 25 percent of starting weight. Individuals in the 4-milligram group experienced a 17.6 percent reduction, while those taking 9 milligrams saw a 23.7 percent drop.

In the diabetes study, patients taking the 12-milligram dose lost an average of 49.6 pounds, or 18.8 percent of their body weight, alongside improvements in blood sugar, cholesterol, and blood pressure. According to Eli Lilly, those in the 4-mg group lost an average of 29.8 pounds, and those taking 9 mg lost an average of 45.4 pounds. Randy Seeley, a professor at the University of Michigan, noted that patients with diabetes generally lose less weight on GLP-1 medications than those without diabetes.

Other Amylin Pathway Developments

Eli Lilly is also testing eloralintide, an experimental drug targeting the amylin pathway. In a phase 2 trial for patients with obesity and type 2 diabetes, a combination of eloralintide and tirzepatide led to an average weight loss of 54.1 pounds at 48 weeks at the highest dose. Eloralintide alone at 6 mg resulted in average weight loss of up to 28.6 pounds, while Zepbound 15 mg alone resulted in 34.4 pounds.

However, side effects led 10.8% to 27% of patients taking both drugs to stop treatment, compared to 2.9% for those on tirzepatide alone. Bikman stated, “A therapy is only effective if patients can remain on it, so tolerability in Phase 3 will be as important as efficacy.” Leerink Partners analyst David Risinger predicts eloralintide products could reach $23.2 billion in annual sales by the end of 2035.
