However, the trend is now spreading beyond these established players. More traditional practices are implementing similar fees, citing financial pressures and a desire to maintain quality care.

The growth is significant. A recent study from Johns Hopkins, Oregon Health & Science University, and Harvard Medical School found an 83.1% increase in direct primary care and concierge practices between 2018 and 2023, rising from 1,658 to 3,036 practices.

Why the Change? The Financial Strain on Practices

Doctors are facing a confluence of challenges. Dr. Sarah Yamaguchi, a gynecologist in Los Angeles, explains that declining insurance reimbursements, rising rent, and the necessitate to fairly compensate staff are all contributing factors. The administrative burden is also immense. She estimates her practice spends significant, unbillable hours navigating insurance requirements – a cost that ultimately impacts patient care.

“They’re becoming more common, because if you’re not a large group, you can’t keep your doors open without it,” Dr. Yamaguchi stated.

This administrative overhead is substantial. One study found that the volume of patient portal messages increased 157% during and after the pandemic, adding to the workload without necessarily translating to increased revenue.

Different Models: Hybrid vs. Concierge

It’s important to distinguish between hybrid and concierge models. Hybrid practices, like those increasingly common in California, typically accept insurance but add a smaller annual fee (often $250-$500) to cover administrative costs and enhanced services. Concierge medicine, usually involves a significantly higher annual fee (ranging from $1,000 to $20,000+) and may involve opting out of insurance altogether, offering a more personalized, “luxury” healthcare experience.

The Telehealth Factor

The rise of telehealth has further complicated the landscape. Whereas offering convenience, it has also increased patient expectations for accessibility and responsiveness. Doctors are now often fielding questions and providing guidance through patient portals, a service that isn’t always adequately reimbursed by insurance.

Impact on Access and Equity

The growth of these models raises concerns about equity. While some patients are willing to pay extra for enhanced access and personalized care, others simply cannot afford it. Experts worry that this could exacerbate existing disparities in healthcare access, creating a two-tiered system.

“The growth of these models may benefit participating patients and clinicians, but it’s important everyone understands the potential impact these practices have on the health care system at large,” says Dan Polsky, co-author of the Johns Hopkins study.

For patients like Mackrandilal, the decision comes down to affordability and value. She ultimately switched physicians to avoid the extra fee, highlighting the importance of finding a practice that aligns with individual financial circumstances and healthcare priorities.

Dr. Ramadan believes that, the future of primary care may depend on increased competition and pressure on insurance companies to adapt. “If it keeps going the way it’s going … I probably will have to charge a fee eventually, but I’m trying not to do that right now.”
