Scientists analyzing eye-tracking data from children participating in memory tests have found that tiny, involuntary shifts in pupil size may reveal subtle differences in how attention operates in young people with attention deficit hyperactivity disorder (ADHD), according to a study published in Scientific Reports.
Pupil Dynamics and ADHD Attention Rhythms
Attention does not function like a static light switch. It fluctuates constantly, holding steady one moment and wandering the next. Researchers from the Indraprastha Institute of Information Technology Delhi set out to determine whether these invisible shifts leave a physical trace in the human eye.
The research team reanalyzed existing eye-tracking data gathered from 50 children aged 10 to 12 in Chile. Out of that group, 28 children carried a diagnosis of combined-type ADHD, while 22 children had no neurodevelopmental diagnosis.
During the experiment, the children played a memory task. They watched dots appear on a grid, tried to remember their locations, and then indicated whether a subsequent dot occupied a previously seen position after a distracting image flashed on the screen. An eye tracker measured each child’s pupil diameter 1,000 times every second across 160 trials.
While pupils naturally adjust to light levels, they also react to cognitive effort, emotional arousal, fatigue, and engagement. Instead of looking only at average pupil size, computer scientist and first author Saumya Yadav and colleagues examined how consistently each child’s pupils responded to identical challenges from trial to trial.

Controlling for participant age, intelligence quotient, and task complexity, the authors’ computational model calculated that unmedicated participants with ADHD displayed roughly a 23 percent increase in trial-to-trial pupil response fluctuation relative to the control subjects.
Within the untreated ADHD cohort, separate trials showed greater differences in pupil reactivity, whereas reactions during the course of a single trial remained more stable. According to the data, unmedicated children who exhibited greater pupil variability also tended to answer questions less accurately during the memory game.

Despite these measurements, the direct comparison between individual participants narrowly missed statistical significance after researchers corrected for multiple tests. Yadav emphasized that the technique cannot currently diagnose ADHD and that the findings remain an early step toward understanding how eye movements correlate with neurological conditions.
The Impact of Medication on Eye-Tracking Metrics
To investigate how pharmacological management affects these ocular dynamics, 17 participants with ADHD performed the memory assessment under two conditions: 24 hours after discontinuing methylphenidate, and subsequently while maintaining their normal dosage schedule.
When medicated, the children’s pupils responded more strongly during the memory task and fluctuated less between rounds. The overall shape and timing of their pupil responses more closely resembled those of the control group.
However, the study authors warned against concluding that medication simply “normalized” attention. Methylphenidate directly affects the physiological systems that control pupil size. Consequently, the experiment could not completely separate whether the observed differences stemmed from improved cognitive attention, the direct pharmacological action of the drug on the body, or a combination of both factors.
Additional limitations included a lack of precise data regarding each child’s specific drug formulation or dose. Furthermore, an average of six months separated the two tests for the medicated group, introducing potential variables such as maturation or practice effects.
