Drinking diet soda daily may significantly elevate health risks, according to recent medical studies linking consumption to increased dementia risk in adults and higher autism diagnoses in male offspring when consumed during pregnancy.
For decades, diet soda has sat comfortably on grocery shelves as the guilt-free choice for anyone looking to slash sugar intake. It promises the fizz and flavor of traditional soft drinks without the metabolic toll. But a pair of detailed scientific studies from academic health centers complicates that reassuring narrative, pointing to potential neurological risks that stretch from late-life cognitive decline back to early prenatal development.
University of Miami Study Links Daily Diet Soda to Elevated Dementia Risk
A fresh analysis from the long-running Northern Manhattan Study (published in the Journal of Alzheimer’s Disease) suggests that drinking diet soda every day connects directly to a higher risk of developing dementia. Led by researchers at the University of Miami Miller School of Medicine, the investigation tracked 947 participants who completed baseline dietary assessments and repeated neuropsychological testing.
Participants in the cohort were, on average, 64 years old at baseline, and more than 20% developed dementia over years of follow-up. After adjusting for demographics, lifestyle behaviors, and vascular risk factors, the research team found that each additional diet soda consumed per day was linked to a 34% increase in dementia risk. Furthermore, participants who drank more than one diet soda daily faced approximately four times the risk of developing dementia compared to those who drank one or fewer daily.

Interestingly, regular soda consumption showed no association with dementia in any multivariable model evaluated by the team. Exploratory analyses also indicated that the elevated risk associated with diet soda appeared strongest among non-Hispanic white and Black participants.
UT Health San Antonio Research Examines Prenatal Aspartame Exposure
The neurological concerns surrounding artificial sweeteners extend far beyond aging populations. Research led by scientists at The University of Texas Health Science Center at San Antonio identified an association between autism diagnoses in boys and daily maternal consumption of diet soda or aspartame during pregnancy or breastfeeding.
The case-control study gathered retrospective dietary recalls from mothers of 235 offspring with autism spectrum disorder and 121 neurotypically developing control subjects. Investigators found that boys diagnosed with autism were more than three times as likely to have been born to mothers who reported consuming one or more servings per day of diet soda, or comparable amounts of aspartame, during pregnancy or lactation. The study found no statistically significant association in female offspring.

Evaluating Consumption Patterns Across Populations
In the Northern Manhattan Study cohort, just more than 2% of participants reported drinking more than one diet soda per day, while just shy of 5% consumed more than one regular soda daily. These numbers align with national data showing heavier use among middle-aged adults and individuals handling conditions like diabetes or obesity.

While the underlying biological mechanisms connecting artificial sweeteners to neurological changes remain under investigation, researchers suggest that caution is warranted. Clinicians recommend discussing beverage choices with patients early in life, while health experts maintain that women should exercise caution when considering diet beverages during periods of heightened offspring vulnerability.
