According to a feasibility study published in the journal Communications Medicine, early surgery for acute eye fluid infections may be more beneficial for patients’ vision than the conventional antibiotic-first approach. Acute endophthalmitis is a severe bacterial infection affecting the fluid and tissues inside the eye. It can develop as a complication of invasive eye procedures, including cataract surgery. While rare, the condition poses a serious threat of sight loss and blindness. Conventional ophthalmology treatment relies on repeated intravitreal antibiotic injections during the early phase, reserving vitrectomy only for cases where antibiotics have not proved effective.
The new trial reveals a contrast in visual recovery. Patients who underwent early vitrectomy achieved a median improvement of 40 letters, with a range spanning 28 to 70 letters. In comparison, patients receiving an initial six-month course of antibiotics saw a median increase of just 13 letters, ranging from 0 to 66 letters, according to the study data. This means the surgical intervention produced over three times the median sight improvement of standard antibiotic treatment.

Patient Recovery Times and Lower Complication Rates
Beyond vision scores, early surgical intervention accelerated patient recovery. Study participants experienced faster visual improvements, bringing prompt relief from a painful condition. Furthermore, prompt treatment alleviated the psychological effects of not knowing whether there would be future sight loss, as detailed in the findings.
Safety metrics also favored the surgical approach. Early vitrectomy patients recorded lower rates of non-serious adverse events at 47%, compared with 68% for the antibiotic group. Incidences of retinal detachment were also lower in the early surgery cohort.
Mahi Muqit, senior vitreoretinal consultant at Moorfields Eye Hospital and associate professor at the Institute of Ophthalmology at UCL, served as lead author on the study. Commenting on the results, Muqit stated, “This important new study shows the potential short-term and long-term potential benefits to patients given an early vitrectomy if they contract endophthalmitis after their eye procedure.”
Because the sample size of 63 patients yielded relatively low levels of statistical significance, researchers emphasize the necessity of broader validation. “As using this intervention at diagnosis shows clear potential to improve clinical outcomes, we now intend to take this forward to a definitive Phase 3 randomised clinical trial that can definitively answer this question,” Muqit said.
