The retrospective cohort study analyzed 20,057 adults aged 20 or older who received a first primary Hodgkin lymphoma diagnosis between 2000 and 2017. Researchers pulled data from 16 population-based registries within the Surveillance, Epidemiology, and End Results (SEER) program. To qualify for the analysis, patients must have undergone chemotherapy with or without radiotherapy and survived for at least 5 years following their diagnosis.

The excess absolute risk calculated by the research team reached 81.9 additional deaths per 10,000 patient-years. Specific demographic groups experienced even higher relative risks. Standardized mortality ratios exceeding 2.50 appeared among Hispanic survivors, non-Hispanic American Indian or Alaska Native survivors, non-Hispanic Asian or Pacific Islander survivors, and individuals diagnosed between the ages of 20 and 39.

Furthermore, the highest excess absolute risks concentrated among survivors aged 60 to 89 years, non-Hispanic American Indian or Alaska Native participants, and patients diagnosed with mixed-cellularity disease. At a median follow-up of 173 months from diagnosis, the 10-year cumulative mortality rate hit 8.0%. Of this total, 2.5% stemmed from non-cancer-related factors, 2.1% from alternate neoplasms, 1.6% from cardiovascular issues, and another 1.6% was attributed directly to Hodgkin lymphoma. By 15 years post-diagnosis, the cumulative mortality rate rose to 15.0%.

Implications for Modern Survivorship Care

The study provides crucial population-based estimates specifically focused on adults treated during the contemporary era of risk-adapted therapy. Previous mortality literature often relied heavily on childhood cancer cohorts, shorter follow-up intervals, or older treatment generations that preceded modern clinical strategies.

These findings suggest that oncology care teams must broaden long-term survivorship protocols. Because cardiovascular and noncancer conditions drive the greatest absolute excess in deaths, follow-up care requires rigorous monitoring of heart health and secondary health screenings alongside routine cancer surveillance.
