Researchers at Peking University School and Hospital of Stomatology in Beijing reported on September 28, 2026, in the International Journal of Oral Science that the master transcription factor RUNX2 directly controls osteoclast differentiation through a newly identified long non-coding RNA, LncRNA-ROD, offering fresh mechanistic insight into cleidocranial dysplasia (CCD).
Gene Mutation and Alveolar Bone Density in Cleidocranial Dysplasia
Cleidocranial dysplasia is a skeletal disorder characterized by delayed permanent tooth eruption, supernumerary teeth, and clavicular defects caused by mutations in or haploinsufficiency of RUNX2. Analyses of alveolar bone samples from patients with CCD reveal increased bone density alongside reduced numbers of tartrate-resistant acid phosphatase (TRAP)-positive osteoclasts. The investigated patient carried a RUNX2 missense mutation (c.557G>C; R186T). Her alveolar bone exhibited greater density compared with controls and contained no detectable TRAP-positive osteoclasts.
Animal models help clarify these human clinical observations. Runx2+/– mice successfully recapitulate CCD-like skeletal abnormalities, exhibiting delayed eruption of the mandibular first molars at postnatal days 11 and 13. These mutant mice show reduced numbers of TRAP-positive osteoclasts as early as postnatal day 5. In laboratory tests using RAW 264.7 cells and bone marrow-derived monocytes/macrophages, the loss of RUNX2 consistently diminished osteoclast marker expression, lowered the count of TRAP-positive cells, and restricted resorption-pit formation.
RUNX2 activates LncRNA-ROD to rescue osteoclastogenesis
Through RNA sequencing, researchers identified LncRNA-ROD as a 1147-bp, three-exon transcript that is enriched in bone tissue and directly activated by RUNX2. Chromatin immunoprecipitation and luciferase reporter assays confirmed that RUNX2 binds directly to the LncRNA-ROD promoter. When investigators overexpressed LncRNA-ROD, it successfully rescued defective osteoclastogenesis in bone marrow-derived monocytes/macrophages harvested from Runx2+/– mice. Conversely, LncRNA-ROD–/– knockout mice phenocopied the dental eruption and bone-resorption defects seen in the broader condition.
Did you know? In isolated osteoclast precursors, deleting LncRNA-ROD increased the expression of fusion markers such as CD44 and ITGB3 while simultaneously decreasing the expression of CTSK and MMP9, ultimately shrinking the resorption-pit area.
Mechanistically, LncRNA-ROD binds to QKI, an RNA-binding protein that normally promotes the degradation of Csf1r mRNA. By sequestering nuclear QKI-5 and cytoplasmic QKI-6, LncRNA-ROD stabilizes Csf1r mRNA and elevates CSF1R protein levels. Knocking down QKI restored osteoclast differentiation in cells derived from both Runx2+/– and LncRNA-ROD–/– mice, and both mutant strains demonstrated resistance to ovariectomy-induced bone loss.
Therapeutic Implications and Limitations for Alveolar Bone Remodeling
The study positions LncRNA-ROD as a central component of a CCD-associated osteoclast regulatory network, indicating that the RUNX2–LncRNA-ROD–QKI axis is a viable target for pharmacological intervention. However, the authors emphasized that current data do not demonstrate whether restoring this axis can reverse existing tooth-eruption defects, as the current mouse models rely on global gene modification rather than targeted alveolar rescue. Osteoclast-specific conditional knockout models and localized alveolar rescue experiments remain necessary next steps.
Additional direct assays of bone formation are required to fully interpret the ovariectomy findings. While these results support the development of RNA-directed therapies using antisense oligonucleotides or small molecules directed at the bone-enriched LncRNA-ROD–CSF1R axis, cell-type specificity must be carefully managed. Validating functional RNA motifs and establishing precise delivery methods to alveolar bone are essential before clinical translation.
What causes cleidocranial dysplasia?
What causes cleidocranial dysplasia?
Cleidocranial dysplasia is caused by mutations in or haploinsufficiency of the RUNX2 gene, which leads to skeletal defects, delayed permanent tooth eruption, and abnormal bone density.
How does LncRNA-ROD affect osteoclast differentiation?
LncRNA-ROD is directly activated by RUNX2 and binds to the RNA-binding protein QKI, which stabilizes Csf1r mRNA and increases CSF1R protein levels to regulate bone resorption and osteoclast formation.
Can current gene therapies reverse tooth eruption delays in CCD?
Current data do not show that restoring the RUNX2–LncRNA-ROD–QKI axis reverses tooth-eruption defects, because existing research relies on global mouse models and localized alveolar rescue has not yet been tested.
