This open randomized trial was conducted at 45 centersin Sweden, Estonia and New Zealand, between September 2017 and May 2023. The researchers recruited 5 020 patients (including 95.4% in Sweden) who have had an acute myocardial infarction (MI) with early coronary angiography confirming the presence of stenosis, and FEVG ≥ 50%. In more than a third of cases it was a myocardial infarction with ST-segment elevation.

The average age of the participants was 65 years, 22.5% were women, 46.2% had hypertension, 14% had diabetes, and 7.1% had a history of myocardial infarction. The cohort was representative of the population of MI patients with preserved LVEF in these countries and participants were, in general, not at high risk of experiencing a new cardiac event. Patients with another indication for beta-blocker treatment for secondary prevention were excluded.

Almost all patients (95.5%) underwent percutaneous coronary intervention (coronary angioplasty) and 3.9% underwent coronary artery bypass grafting. They were then randomly divided into two groups. In the first, participants received beta-blockers for the remainder of their hospital stay and after discharge: from metoprolol for 62.2% of them (median initial dose: 50 mg/day; median target dose: 100 mg/day) or bisoprolol for 37.8% of them (median initial dose: 2.5 mg/day; median target dose: 5 mg/day). In the second (control group), they did not receive beta-blockers (but not even a placebo).

Upon leaving the hospital, nearly all patients in both groups were taking a statin (99% and 98% respectively), aspirin (98% and 97% respectively), and a P2Y12 receptor inhibitor (96%); finally, 79% and 81% took an ACE inhibitor or an ARA2 respectively.
